I am writing to make a request under the Freedom of Information Act 2000. I would be grateful if you could provide the following information:
- Total number of clinical negligence claims and incidents recorded under the cause code ‘Failed Sterilisation’ by notification year, covering financial years 2021/22, 2022/23, 2023/24, and 2024/25 to date.
- For each claim and incident recorded under the cause code ‘Failed Sterilisation’ (as identified in Question 1), please provide:
a) The location of the incident
b) The injury code(s) and nature of injury
c) The clinical specialty or specialties involved
d) The incident description as recorded in the free text field on CMS
e) The incident date (year)
f) The current status (open/closed)
