The following information was requested:
I would like to know how many completed cases of clinical negligence or admitted cases had a failing of the Electronic Prescribing and Medicines Administration listed as a cause please.
I would like this data for the last four years please.
One more thing, could I have that broken down by trust and amount please?

I would like to see a breakdown of all the “medication errors” for the last four years please.
This should only be for hospital trusts pls.
I would like to see this broken down by successful and unsuccessful claims, or ones that have simply been logged but not determined yet.
I am also interested to know the numbers for “medication errors” that fall under the “non-clinical claim” category, this should also be broken down by trust pls.
