Please can I kindly request the following:
1. How many incidents were reported to NHS Resolution under the Early Notification Scheme for each individual Trust in the North of England, broken down by Trust and financial year from the financial year 2019/2020 to date.
2. How many of these reported incidents progressed to a formal claim broken down by Trust and financial year from the financial year 2019/2020 to date.
3. How many of these reported incidents progressed to a finding of breach broken down by Trust and financial year from the financial year 2019/2020 to date.
4. How many of these reported incidents progressed to a potential claim broken down by Trust and financial year from the financial year 2019/2020 to date.

Please may I have the data both broken down by year and also in a way that you think would provide more meaningful data, as you have not stated in which way the data will be provided that would make it more meaningful.
