Under the Freedom of Information Act, we would like to know how many claims, serious incidents, untoward deaths (over the last 5 years) have occurred in NHS Trusts that were as a direct or indirect result of ‘missed instructions given to secretaries’ (by clinicians) to follow or chase up something – specifically: actions not picked up/chased up from the clinical correspondence.
It would be helpful to understand those involving diagnostic tests, which either did not take place and/or where missed causing a delay in diagnosis causing disease progression/death and other types of missed actions i.e. not pertaining to diagnostic tests or receipt of diagnostic results.
