I am looking to receive any records relating to claims since 2013 around NHS hysteroscopy procedures undertaken within gynaecological departments, including hysteroscopic myosure procedures, resulting in any adverse outcomes (including uterine perforation or bowel injury etc.) which have caused a claim against the NHS to be raised.
