The following information was requested:
To help us examine harm and negligence from delays in care (and some associated issues) can we please receive the claims data by year back to 2010 for each NHS trust using the following parameters.
Cause codes:
- Delay In Performing Operation
- Fail / Delay Treatment
- Fail To Follow-Up Arrangements
- Fail/Delay Admitting To Hosp.
- Failure To Perform Operation
- Failure/Delay Diagnosis
- Fail/Delay Referring To Hosp.
- Fail/Delay Avail Op Thtre
- And on a separate sheet if possible
- Inappropriate Discharge
- Sexual Abuse
- Injured By Another Patient
- Injury To Others By Patient
For each cause code, can I get the primary and secondary injury data, and successful claims, damages paid and costs for each.
