The following information was requested:
I would like all the information you hold in its most granular form going back 10 years. I am particularly interested in claims related to anaesthesia, pain medicine/management, interventional radiology, intensive care (adult and paediatric).
For each of these areas it is important that all the information you hold is provided and not filtered.
I want to see dates of incidents, narratives of claims where available, details of the operations or procedures involved, any available details you can share about the patients affected, and most importantly the amounts paid in settlement.
