I am writing to request information under the Freedom of Information Act.
Can I please request the following information for each of the following years: 2017, 2018, 2019,2020,2021,2022, 2023, 2024,2025 (up to the most recent date available):
- Number of payouts made for cases where errors with medication were made (e.g., patients given wrong drug by mistake or wrong dosage; medication administered incorrectly; errors with medication on children’s wards, etc.
- Amount of compensation paid for cases where errors with medication were made, plus legal fees
- a breakdown of the compensation cases and amounts by the type of error, if possible
- number of cases and amount paid out to claims where the patient died due to a medication error
