The following information was requested:

1. How many clinical negligence claims involving amputation were paid out by the NHSLA in the year 2020/21 what was the monetary value of payments made against those claims, irrespective of when the claim was made? For each amputation please list the body part or limb involved. Please give the three NHS Trusts against whom the most successful claims were paid out in 2020/21 and how many claims related to each of those trusts.

2. How many clinical negligence claims involving blindness were paid out by the NHSLA in the year 2020/21 and what was the monetary value of payments made against those claims, irrespective of when the claim was made? Please give the three NHS Trusts against whom the most successful claims were paid out in 2020/21 and how many claims related to each of those trusts.

How many clinical negligence claims involving cosmetic disfigurement were paid out by the NHSLA in the year 2020/21, what was the monetary value of payments made against those claims, irrespective of when the claim was made? Please give the three NHS Trusts against whom the most successful claims were paid out in 2020/21 and how many claims related to each of those trusts.

Bloodstream infection

The following information was requested:

In the last 5 financial years (2017-2021), please provide details of negligence/financial claims made against NHS Trusts/clinical staff members or incident reports made for patients acquiring a bloodstream infection due to care they received in NHS (excluding blood transfusion)? Expressed as number of incidents and percentage of all claims/incidents (or expressed in any alternative data format which you have available)?

What number of these were associated with centrally inserted intravascular catheters (and percentage of all claims)?

How many of these were associated with (and percentage of all claims)?

Did any of these incidents lead to patient death?

What was the average cost per case, range of cost, and the total cost of all these cases over 5 years to NHS (and percentage of all claims costs in the same time period)?

The following information was requested:

My FOI request relates to the NHS Resolution claims that have been submitted in the seventeen-year time period from 1st January 2005 to 1st January 2022.

Can we be provided with the details of the location (county or region) and clinical specialty to the extent that it relates to the following specialities:

  • Orthopaedic
  • Gynaecologist
  • Ophthalmologist

If providing the data for the dates above would be an excessive burden to the Freedom of Information Service, I would be grateful for the maximum possible that you consider to be feasible from 2022 backwards.

For:

Gloucestershire Royal Hospitals NHS foundation trust and
Gloucestershire Health and Care Trust (previously 2gether NHS Foundation Trust and Gloucestershire Care Services NHS Trust)

Could you also provide the average or expected figures for an average or typical NHS trust for the same 10 year period.

The motivation is to provide a case for the trust to purchase access to a radiological clinical decision assistant knowledge base (e.g. statdx.com) with the aim of reducing radiological errors and thus litigation costs.

I would like to know what indemnity you provide to NHS Hospitals regarding treatment of private patients in NHS hospitals. My main inquiry is that what indemnity/insurance is provided for non-consultant doctors when they treat private patients free of charge in NHS hospitals.
