I would like to make an FOI request for the following information please:

1. What is the total cost of litigation settlements to the NHS on an annual basis as a result of medication errors? To include Adverse Drug Effects and Death.
2. Is this data available down to Trust level?

If it isn’t too much of an admin burden, 3-5 years data would be amazing. Failing that, 12-24 months would be fine.

#1 Please supply the number of assault claims relating to a mental health location received in each financial year from 2016/17 to 2020/21, broken down by Primary Injury.

#2 Please supply the number of assault claims relating to a mental health location received in each financial year from 2016/17 to 2020/21, broken down by staff occupation.

#3 Please supply the total cost of assault claims relating to a mental health location in each financial year from 2016/17 to 2020/21, broken down by (a) damages, (b) defence costs and (c) claimant costs.

Impotence, Incontinence

The following information was requested:

How many clinical negligence claims involving an injury code of (i) loss of sexual function, or (ii) impotence, were paid out by the NHSLA or NHS Resolution in the combined last three financial years? (18/19) (19/20) and (20/21)?

What was the monetary value of payments made against those claims that were paid in those three combined financial years, regardless of when the incident took place or the claim was lodged?

How many clinical negligence claims involving an injury code of (i) incontinence were paid out by the NHSLA or NHS Resolution in the combined last three financial years? (18/19) (19/20) and (20/21).

What was the monetary value of payments made against those claims that were paid in those three combined financial years, regardless of when the incident took place or the claim was lodged?

Shoulder Injury Related to Vaccine Administration

The following information was requested:

The extraction of data may depend on the categorisation used by your system, and whether it codes conditions in the same way as the NHS, the WHO, or some other method.

In the first instance, a good comparison with the situation in the USA would be to search for “SIRVA” [Shoulder Injury Related to Vaccine Administration”].

If that were not available, then Rotator Cuff Syndrome and perhaps Bursitis of the Shoulder, could be identified, these being a couple of the conditions lumped together under the SIRVA umbrella.

These consequences could potentially indicate faulty injection technique, rather than recognised systemic reactions or more remote effects such as Guillain-Barre Syndrome or Cardiomyopathy.

Behavioural Assessments

The following information was requested:

How many cases following behavioural assessments have found individuals in your organisation’s experience/expertise of being assessed as having a neurodiversity. By that I mean autistic spectrum related, ADHD, dyslexia, etc.

If you were able to share information individually from the three different occupations i.e. doctors, dentists and pharmacists.

• Total numbers that had a positive diagnosis/Association against the total number assessed

• Percentages.
