Surgeons referred to Advice service
The following information was requested:
Thank you for your email of 21 February 2021 in which you requested further information from the Practitioner Performance Advice (Advice) service following your previous FOIA request dated 12 January 2021.
Firstly, we would like to acknowledge that there is a slight correction to be made to the information shared with you in January following a data quality review of our cases. Previously, we identified that there were 821 requests for advice about practitioners who were surgeons between 1 April 2015 and 31 March 2020 (financial years). A recent internal review of Advice cases and the criteria used to provide you with the information flagged that this number is actually 820. Please do accept our apologies for this. We have provided revised figures to readdress your initial FOIA request in Appendix A. It may be helpful to note that information we hold in relation to live cases can be subject to change owing to case updates from the parties with whom we are engaging and other steps we may take periodically to ensure our records are complete and accurate.
For the FOIA request dated 21 January you have asked for additional information for the years 2015-2020 about the number of surgeons who were a subject of Advice cases. Please note that some information we hold may be listed as ‘not known’ for one of two key reasons: this information has chosen not to be disclosed to Advice by the employing organisation or practitioner or that we are awaiting confirmation of the information from the organisation/individual practitioner prior to recording this on our case management system to ensure it is accurate.
For this latest FOIA request (received on 22 February 2021) you would like to know:
1. An ethnicity breakdown for these surgeons
2. Breakdown by grade – Consultant, Trainee or other non-training grade.

IT Hosting Services
The following information was requested:
I wish to submit a request to the organisation around their hosting contract(s) with 3rd party providers.
The type of contract I wish to see is below:
1. Dedicated hosting- Managed environment
2. Co-Location- hosting allows a business to still own their own server equipment; however, instead of storing it in their own data centre, they instead are able to store it in rented space in a colocation hosting centre.
3. Cloud Hosting- Cloud hosting services provide hosting for websites on virtual servers, which pull their computing resource from extensive underlying networks of physical web servers.
Not all of these will be applicable to the organisation.
For the different types of hosting services, can you provide me with the following information:
1. Type of hosting – Dedicated, Co-Location, Cloud Hosting, Other?
2. Who is the supplier of the contract? If possible can you also provide me with the name of the vendor, if applicable?
3. What is the annual contract value for each contract?
4. What type of cloud environment?
Private Cloud- a distinct and secure cloud based environment in which only the specified client can operate.
Public Cloud – where cloud services are provided in a virtualized environment, constructed using pooled shared physical resources, and accessible over a public network such as the internet.
Hybrid- integrated cloud service utilising both private and public clouds to perform distinct functions within the same organisation.
5. What is the original start date of the contract agreement? If there are more than one contract please provide me with the start date for each contract.
6. What is the actual expiry date of the contract agreement? If there are more than one contract please provide me with the expiry date for each contract.
7. When will the organisation plan to review this contract? If there are more than one contract please provide me with the review date for each contract.
8. What is the contract period in years? Please include whether the agreement has any extension periods?

Medication Errors
The following information was requested:
In a previous FoI response [Ref: FoI_2999] you provided details on the number of claims and the payments made to people issuing claims under the category ‘medication errors’. Could you please provide me with similar information, specifically:
1. I would like to know the number of successful claims paid out by NHS Resolution/NHSLA in each of the last five financial years (15/16) (16/17) (17/18) (18/19) and (19/20) as well as the total amount of compensation paid to these claimants in each financial year?
2. Could you also supply me with tables showing the aggregate totals for medication error claims for the five financial years which give (a) the number of successful claims, (b) damages paid, (c) defence costs paid, (d) claimants cost paid and (e) total paid broken down by the speciality.
3.
