The following information was requested:
I want to submit a request for some information from the organisation, in relation to their contract’s register.
The contract register should display all the organisations existing/live contracts I would like the register to display the following columns/headings:
1. Contract Reference
2. Contract Title
3. Procurement Category
4. Supplier Name
5. Spend (Total or Annual)
6. Contract Duration
7. Contract Extensions
8. Contract Starting Date
9. Expiration Date
10. Contract Description [Please provide me with as much detail as possible.]
11. Contact Owner (Full contact details if possible.)
12. CPV codes/ProClass
IMPORTANT
1. For those organisations planning to make an exemption around spend, the spend information I have requested is an overall figure and I am not requesting a complete breakdown of services relating to the spend.
2. If the organisation has a CRM system or a similar system there should be a facility to download and extract contract data.
3. You may forward me a Web link to a portal to download the contract register, please make sure all of the organisation’s contracts are provided as doing prior research I have found that most organisations have only uploaded a small portion of all of their contracts.
Please do not think that this is the only information I require if you could provide me with more information that would be great.
Contract Data/API Contact Details
13. Can you also provide me with contact details of the person responsible for the actual contract’s register or someone responsible for API? [Name, Job Title, Telephone, Email Address] At the very least provide me with their actual job title.
(Meaning of API “a set of functions and procedures that allow the creation of applications which access the features or data of an operating system, application, or other service.”)
You may have received the same request in the past. I am seeking the latest contract register which is not available in organisations website. Hence sending this request again.

Testicular Torsion
The following information was requested:
I am looking for information how many settlements there were for testicular torsion from January 2007 to June 2019 and in what hospital department the claim was initially against.
I understand that this may not be feasible as most litigation would be against the entire trust. IT would be useful for myself as I am currently researching ways to reduce missed torsions in our emergency departments nationally. If possible I would be very grateful for this data.

Occupational Therapy
The following information was requested:
My FOI request relates to the National Health Service Litigation Authority (NHSLA) and NHS Resolution claims that have been submitted in the 20 year period between 10 Sept 2000 and 10 Sept 2020. The purpose of this request is to explore NHSLA/NHSR submissions which involve, include, or are submitted by, or about, Occupational Therapy / Occupational Therapists as a staff group.
If providing the data for the dates above would be at excessive burden to the Freedom of Information Service, I would instead be grateful for details of the last 10000 claims to
NHSLA/NHSR.
If providing data for 10000 claims is still not feasible for your FOI office, I will be grateful for the maximum possible that you consider to be feasible, starting from the most recent.
Specifically:
How many incidents occurred?
When was the claim/incident submitted?
Where did the claim/incident occur? You might have this in the form of primary care, secondary care, community, wards, outpatients, emergency department etc. You might also have this by geographical region, which would be helpful to know.
What was the description of each claim? If no detailed description can be provided, what were the most common allegations?
What was the NHSLA/NHS category for each incident? You might have these as examples medical device/equipment, patient accident, treatment, procedure, disruptive aggressive
behaviour etc.
What of these have been classified of causing no harm/minor/moderate/major harm/life threatening harm?
What were the claimant demographics? For example, age, gender, ethnicity
Which have been shared/discussed with the patient and/or family?
Which of these claims have been settled, and which are not settled?
What were the most common reasons for lost claims?
What were the most common reasons for settled claims?
What were the financial costs related to settled, lost, and defended claims? This includes costs incurred and damages paid
What was the grade of Occupational Therapist involved? You might have this according to Student/Band 5,6,7,8 etc.

Malnutrition
The following information was requested:
How many claims have been settled in the last five financial years up to and including 2019/20 where one of the injuries is recorded as malnutrition? Please state how many were (i) settled
with no damages and (ii) settled with damages paid?
For those where damages were paid please state the (i) total of these damages, (ii) the defence costs paid and (iii) the claimants’ costs paid?

Claims in excess of £5million
The following information was requested:
This question relates to all settlements that have been made by the NHSLA and NHS Resoluti 0n where the settlement figure of compensation was equal to or in excess of £5million,
or the total budgeted payment was estimated to be equal to or in excess of £5million (this is for a staged payment where the initial payment may be less than £5m but over the life of the claim you have budgeted to pay out more than £5m).
For each settlement please provide the trust name, damages paid and/or total damages budgeted to be paid, injury and speciality.
Please could this data be provided for the last ten years so starting with the 2010/11 financial year up to and including the 2019/20 financial year.

Endoscopic retrograde Cholangiopancreatography (ERCP)
The following information was requested:
I would like to request the medical claims data between 2000-2020, regarding Endoscopic retrograde cholangiopancreatography (ERCP) and duodenoscope procedures within gastroenterology speciality, causes related to failed infection control policy/hospital hygiene, cross-infection or bacterial infection (cause CNST), resulting in hospital-acquired infection (injury CNST).

appreciate the constraints of sensitive personal data).

4. The no of claims/cases involving obstetrics.
5. The details of these cases including settled and outstanding claims (subject to constraints in 2).
6. The no of claims/cases involving gynaecology.
7. The details of these cases including settled and outstanding claims (subject to constraints in 2).
8. The no of claims involving consent in cancer.
9. The details of these cases including settled and outstanding claims (subject to constraints in 2).
10. The no of claims involving consent in women’s cancer (breast, uterine, ovarian, cervical, vulval).
11. The details of these cases including settled and outstanding claims (subject to constraints in 2).
We appreciate that the coding of causes is completed by claims managers at the time at which a claim is notified and that this may change during the course of a claim as the settled facts
emerge.
