I am writing to request information under the Freedom of Information Act, please can you provide the following:
1. Number of claims made for ‘failure/delay in diagnosis’ and/or ‘wrong diagnosis’ for prostate cancer for the following years and by NHS trust 2019-2020 to 2022-2023
2. Value of claims successfully closed or settled where the Primary Cause is failure/delay in diagnosis or wrong diagnosis.

Stillbirths, Cerebral Palsy, Brain Damage (FOI_6428)
The following information was requested:
Follow up to – FOI_6392
Apologies, to clarify, could we have it as a collective between those 10 years by trust.
Attached (FOI_5320) is an FOI we ran a couple of years ago that has it broken down by trust with the totals over the 10 years added together (see table 3 – this is the table we’re looking for please). Could we have a repeat of this just with the latest years (2012/13 through to 2022/23 financial year) please.
Urology (FOI_6430)
The following information was requested:
Please would I be able to request data on the number of litigation cases relating to ureteric stents nationally? I would need to know timeframe cases cover, type of complaint/problem, any regional breakdown of numbers and financial payouts/total costs of claims process.

I would imagine the claims I am looking for would come under any of the following codes:
– Cause codes: Delay in performing operation, fail/delay treatment, fail to follow up arrangements
– Injury codes: additional/unnecessary op, infectious disease, renal damage/failure, sepsis
– Speciality codes – urology, radiology
Despite not being able to be specific the data you have offered to provide would be helpful, thank you. I would be interested in data from the last 10 financial years.

Claims_Supplementary Annual Statistics (FOI_6431)
The following information was requested:
1. Reference: Tab: NHSR – 10 in the “Supplementary-Annual-Statistics-2022-23-3.xls” file found here. The tables on this tab contain average damages values for settled claims by specialty. For each scheme, for the Obstetrics CP/BD category, can you please provide the average damages values separately for claims settled as
a) PPOs and
b) lump sums for each settlement year?
In addition, for the Obstetrics CP/BD category, can you please provide the corresponding number of settled claims separately for claims settled as a) PPOs and b) lump sums for each settlement year?
2. Reference: Tab: NHSR – 1 in the “Supplementary-Annual-Statistics-2022-23-3.xls” file found here. The tables on this tab contain payment totals split by damages values bands. For each scheme, can you please provide the payment totals split by damages values bands by payment year separately for claims settled as
