Just seen this latest publication of yours. I’m wondering if you could provide me with the data that contributed to the ‘anaesthetic errors’ please? NHS Resolution – Did you know? Paediatric medicine errors

I am referring to the page headed “Which specialty results in the most claims?” The text immediately below this heading includes ‘anaesthetics’ as a common medical specialty in which errors occur.
My query is essentially an elaboration on this data, i.e. those claims originating from anaesthesia as a specialty in which a drug error occurred. (…) I wondered if you could comment on which drugs are commonly mis-prescribed; what age of child tends to be most susceptible to such errors; whether errors are more common outside of a tertiary setting (versus a district general hospital say); and whether errors are associated with specific surgical procedures.

2. Of those incidents reported to NHSR’s Early Notification Scheme, the number which have met the scheme criteria
3. Of those incidents which have met the scheme criteria, the number where NHSR’s investigation of the incident has been completed
4. Of those incidents which have met the scheme criteria, the number where NHSR’s investigation of the incident has begun, but has not yet been completed
5. Of those cases where the investigation has been completed, the number in which it was determined that the care provided had fallen below a reasonable standard
6. Of those cases where the investigation has been completed, the number in which an admission of liability has been made
7. Of those cases in which it was determined that the care provided had fallen below a reasonable standard, the number in which any financial compensation has been provided (i.e., an interim award of damages or a final award)
8. Of those cases in which it was determined that the care provided had fallen below a reasonable standard, the number in which a final settlement has been reached resulting in the award of a lump sum or PPO

Missed-scaphoid-fractures (FOI_5910)
The following information was requested:
(…) I have chosen to write an assignment on the incidence of missed scaphoid fractures and the long term outcomes.
If possible, I would like to know if you have any details over the last 10 years of the monetary cost of missed scaphoid compensation claims. I have found one paper from the NHS litigation team, but the monetary payouts aren’t available on there.
If I could access this it would be very helpful; (…), and an e-copy would be preferable.

Vaccine Administration (FOI_5906)
The following information was requested:
I would like to request the information below – please do let me know if you require me to provide any clarification to enable you to find the information.
A list of the claims paid out, relating to the administration of vaccines, for the last 5 years. This list should include at least the amount paid out and the reason/claim it was paid out for. By “administration”, I mean situations where the claim arose as a result of a vaccine not being administered/injected into one’s body properly.

Orthopaedic Surgery (FOI_5904)
The following information was requested:
Under the specialty code ‘Orthopaedic Surgery’, I would like to search all cause codes and all injury codes from financial year 2017/18 to financial year 2022/23 (5-year period).
I would greatly appreciate the provision of data in this search pertaining to the clinical claim, non-clinical claim, claimant costs, defence costs, damages paid, incident date/year, notification date/year, incident description field, location, periodical payment order, and unsuccessful claims.
