South quarterly network meeting – November 2025

The Safety and Learning team hosted the South quarterly network meeting on Tuesday 25 November 2025, chaired by Caroline Latham-Parker, Safety and Learning Lead – South.

Presentations and speakers included:

  • Ingrid Henderson (Associate Safety and Learning Lead – South, NHS Resolution) delivered a national and regional overview on the learning from venous thromboembolism (VTE) claims
  • Victoria Wilkins (Associate Safety and Learning Lead – North, NHS Resolution) presented the deep dive into maternity claims and VTE and the resource called ‘AIM: reducing VTE in maternity care’
  • Olufunsho Otenaike (VTE Patient Safety Lead, Maidstone and Tunbridge Wells NHS Trust) shared the trust’s experience of capturing VTE risk assessments, an improving picture
  • Natalie Smith (Associate Director of Healthcare Legal Services, North Bristol NHS Trust) presented the trust’s work on supporting staff affected by a significant adverse event, and designing a wellbeing pathway
  • David Beattie (Associate, DAC Beachcroft LLP) and Edward Duckworth (Senior Associate, Bevan Brittan LLP) presented case studies and learning points from VTE claims
  • The Q&A session was hosted by Joanna Lloyd (Partner and Head of NHS Health and Care, Bevan Brittan LLP)
  • This was followed by attendees moving into smaller breakout groups for discussion on the presentations provided
