The following information was requested and clarified:

I am writing to request information under the Freedom of Information Act 2000.

Please provide aggregated, anonymised data relating to claims involving skin surgery (excluding cosmetic surgery), which can include skin surgery (performed by dermatologists, plastic surgeons, other specialties etc) handled by NHS Resolution over the past 10 years (ie. 01/01/2016-31/12/2025.

In Particular, we would like to request

1. Overall Claims

* Total claims related to skin surgery excluding cosmetic surgery
* Claims that were settled with damages paid
* Claims that were settled with no damages claimed
* Claim frequency by year
* Trends over time in claim volume and cost
* Total sum of damages paid

2. Individual claim details:
* Type of skin surgical procedure – eg. Punch Biopsy, Curettage and Cautery, Shave Biopsy, Incisional Biopsy, Excision Biopsy, Wide Local Excision, MOHS Surgery
* Reason for claim eg. Failure/delayed diagnosis, fail/delay treatment, wrong site surgery, inappropriate treatment, failed to follow up arrangements, failed to warn informed consent
* Contributing factors eg. Procedural error, Diagnostic error, Treatment-related complication; Psychiatric/psychological damage, cosmetic disfigurement, tissue damage, burns, fatality, additional/unnecessary operations, advanced stage cancer, pain, scarring, cancer
