Under the Freedom of Information Act 2000, I kindly request the following data for obstetric negligence claims resulting in neonatal brain injury (including hypoxic–ischaemic encephalopathy and cerebral palsy), for claims notified and settled between financial years 2017/18 and 2024/25:
The age of the child at claim notification is categorised into the following bands:
0–2 years
3–5 years
6–11 years
12–15 years
16–18 years
Over 18 years
The age of the child at settlement, using the same age bands.
Median and interquartile ranges for:
Time from incident (birth) to notification of claim.
Time from the incident (birth) to the settlement of the claim.
If available, anonymised breakdown by ethnicity of the mother/child.
Sexual assault claims (FOI_7507)
The following information was requested:
Request FOI_7118 – about compensation for sexual assaults in May this year. This had data up to the financial year 2023-24.
- Could you update that with the last financial year’s figures – 2024-25 – for tables 1,2,3,4 & 6?

The following information was requested:
1. In (i) 2024/2025 how many claims has the NHSLA paid out where the injury code has
been removal of testicle and what was the total value paid out in damages for such
claims?
2. Please state how many “removal of testicle” claims were linked to each separate type
of cause code e.g. (wrong diagnosis, operation on wrong part of body etc).

Assault claims (FOI_7506)
The following information was requested:
I noticed that NHS Resolution answered a freedom of information request last month about compensation claims over the past five years, ref. FOI_7377.
Would it be possible for you to provide me with a version of Table 2 from that response, which covers only the past three financial years? That would be 2022-23, 2023-24 and 2024-25?
That table covers the number of claims and incidents received where any cause is ‘assault’ and liability type is ’employers liability’, broken down by trust.

CNST claims at Lewisham and Greenwich NHS Trust (FOI_7504)
The following information was requested:
Under the Freedom of Information Act 2000, I am requesting information on clinical negligence claims.
Please provide a list of all clinical negligence claims over the treatment of patients at University Hospital Lewisham where a compensation payment was made to a claimant in the most recent full financial year.
For each claim, please provide the following information in a spreadsheet:
– The unique reference number for the claim.
– The year the incident occurred.
– A narrative summary of what went wrong in the patient’s care. I am specifically requesting a brief, anonymised paragraph that describes the sequence of clinical failings. For example: “A delay in reviewing a post-operative patient with signs of sepsis led to a missed diagnosis and the need for life-saving intervention” or “A misinterpretation of a spine scan resulted in a failure to diagnose a fracture, leading to neurological deterioration.”
– The amount of damages paid to the claimant (£).
– The date the payment was made.
I recognise your overriding duty to protect patient confidentiality under Section 40 of the FOI Act and UK GDPR. I request that you provide these narrative summaries in a form that is fully anonymised and does not permit the identification of any individual patient. If providing a full narrative is not possible for certain cases without risking identification, please provide the most detailed categorical description available (e.g., “failure to diagnose and treat infection postsurgery”) as a fallback.
