The following information was requested:
I would like to make a freedom of information request.
I am doing a research project investigating call-off contracts in the public sector. I have
identified a potential call-off contract awarded by NHS Resolution, but I can’t find details of the
framework agreement it was awarded from.
I have attached an Excel file that contains the information I am looking at. The last two columns
(“Title of framework used” and “Further framework info”) is where I am missing information.
Please could you provide the name of the specific framework agreement or DPS used here. If
there is any further info which you think would help me locate the framework agreement (e.g., a
link to the framework’s Contract Finder or FTS listing, the framework provider, or a widely-used
reference number such as CCS’s RM codes), please use the final column for this.
Please note that I have identified this contract as a possible call-off contract, so there is a
chance it is not a call-off. It could be, for instance, procured directly (without being called off
from a framework agreement), or could be itself a notice of the establishment of a framework
agreement. Therefore I would kindly ask you to specify in this instance what kind of
procurement was used in the “Title of framework used” column.
I have provided the title, description, the publication date, and procedure type used to award the
potential call-off, as well as a URL link to the call-off in question and a unique reference ID for
the potential call-off. Please let me know if there is anything else you need to complete the

Foreign body and Wrong site surgery (FOI_7428)
The following information was requested:
1. Please could you provide me with the total number and the total value of claims paid out by the NHSLA for the financial year 2024/2025 where ‘surgical/foreign body left in situ” is one of the grounds for claiming showing damages paid and costs paid.
2. Please also provide the individual amounts of compensation paid for the five biggest awards and please state what the foreign body was?
3. For the last financial year 2024/2025 how much compensation has been paid out in total by the NHSLA as a result of wrong site surgery? (i.e. operating on the right leg instead of the left etc).
4. For the last financial year 2024/2025 what was the largest single payment made for compensation as a result of wrong site surgery?
5. For the last financial year 2024/2025 what was the total number of claims paid out as a result of compensation for wrong site surgery? Also provide a list giving a brief explanation of each wrong site surgery claim. E.g. wrong eye, wrong knee, wrong kidney etc.
Please note that this question refers to claims paid out in 2024/25 regardless of when the claim
was lodged or when the incident took place

A breakdown of costs and compensation payments by Trust as requested, or a full explanation as to why this disclosure is not being made so that I can formulate an appeal if necessary
A further breakdown of the disclosed claims to differentiate between bodily harm and murder, or a full explanation as to why this disclosure is being withheld, to allow me to formulate an appeal if necessary
Please could you also break down the number of claims by year, as requested. Or please provide an explanation as to why they been amalgamated into an overall figure.

Miscarried babies (FOI_7419)
The following information was requested clarified:
I am writing to request information under the Freedom of Information Act 2000. I would like to obtain the following information:
– The number of cases in the United Kingdom where miscarried babies have been lost, discarded, or contaminated for the past five years.
– The number of these cases sent to NHS resolution, the numbers of these cases which have undergone action by NHS resolution, the number of successful cases (where compensation is awarded)
– A geographic/regional breakdown of these cases by trust and or hospital
– The amount of money in compensation claimed for these cases for the past five years.
– The total amount and a list of each individual amount per case.
– A geographic/regional breakdown of these cases by trust and or hospital
Also, by United Kingdom, I mean Wales, England, and Northern Ireland as I have asked Scotland’s National Services Scotland for an FOI.

and 5th April 2025. - The number of medical negligence claims pertaining to dental care which have been lodged
