At our regional quarterly network meetings throughout September, Louisa Bradley (Safety and Learning Lead – North) shared the findings from our recent review of suicide-related claims involving mental health services in England.
The session explored learning from 960 suicide-related claims identified from 2018 to 2025, helping organisations understand common themes and opportunities to strengthen patient safety and reduce future harm.
